You may notice a new entry in your NHS App referring to Segmentation, Patient Need Groups (PNGs) or Johns Hopkins.
What is Segmentation?
Segmentation is a national NHS initiative designed to help healthcare providers understand patients’ health needs more accurately. Developed by Johns Hopkins University, it groups patients into 11 categories based on their overall health and wellbeing. This helps practices provide more personalised care and ensure that patients receive the right support at the right time. If you see this entry in your NHS App, no action is needed from you.
What are Patient Need Groups (PNGs)?
Patient Need Groups categorise patients based on the complexity and intensity of their healthcare need, not just their medical conditions. The PNG system considers a wide range of information, such as long-term conditions, how often a person needs healthcare, and the level of support they are likely to require. Understanding these groups helps explain why different patients may need different types or levels of care.
How do PNGs work?
- Everyone is different: some people are generally well, some have a few long-term conditions, and others have more complex health needs.
- The PNG system places people into one of 11 groups, depending on how much care and support they are likely to need.
- Your group may change over time if your health changes.
The 11 Patient Need Groups
- Green (PNG 1–4) You are generally healthy or have low health needs. You may only require routine check-ups and advice on staying well.
- Amber (PNG 5–9) You have some ongoing health needs. This group may include, for example, people living with mental health conditions or those who are pregnant. You might need occasional support from your care team to help manage your health.
- Red (PNG 10–11) You have more complex health needs or difficulties with mobility. You may require more regular or intensive support from your care team.

They are an additional supporting tool that help healthcare professionals better understand your needs and help to make decisions about your care.
