How to register for the NHS App
NHS App is a great online service which allow you to book appointments, order repeat prescriptions, view your medical records and more. The service is open 24/7/365 and can be accessed from your home PC, Tablet or Mobile phone.

Creating a NHS App account and linking with the Surgery
Watch this video to find out how to register with the NHS App and verify your Identity at home using your photo ID.
Once you have finished your registration you will be set up for the basic default account, where you can view some of your medical record like allergies and medication and you can book a limited amount of appointments
To view historic medical records and test results please follow the steps for Online Services Applications below.
*Please note that the surgery currently only offers online bookings for Blood Tests and some Nurse Appointments
If the verfication of your photo ID does not work on the app, and/or you prefer to have your identity verified at the surgery first follow the steps from the Online Services Application below.

Online Services Application
Step 1. Read, Print and Complete This Form
Step 2 Visit the practice in person and hand to reception your completed application form. Please confirm with the reception team that you've read the information leaflet.
Please note that we also have copies of the application form and information leaflet at reception if you are unable to download, complete and print it.
Step 3 Show your two forms of Identification: One photo ID such as passport, drivers licence, freedom pass and one form of ID with your home address on such as a recent utility bill, bank statement. Full List of acceptable ID
Once you have completed the above steps, the receptionist will generate an Online Services Pin Document which you need in order to sign up for a NHS account without a need of ID
Once you have completed the registration online you will have access to the basic functionality (ordering medication and managing appointments). If upon application you requested access to blood test results and access to your medical record the additional access will be granted within 2 to 4 weeks. ( If you are a newly registered patient this may take longer if the practice has not received your full medical record at the time)
Information for patients with NHS App moving surgeries
Patients that have pre existing NHS App accounts with additional access, registering with us, will need to repeat the process of Online Services Application at our surgery.
Once the form has been completed and the surgery has received your medical record from your previous surgery, the access requested will be granted.
